A CTSM Program Enrollment form must be on file at the CTSM office prior to any records being archived. Once session attendance has been verified, you will be required to take the appropriate quizzes and pass with a score of 70% or better in order to receive credit for the course.

An Archive Research Fee of $125 for each conference researched is due at the time of request.
