Credit can only be awarded for sessions attended at learning events within the past three years. Please ensure that the sessions you are requesting credit for fall within this time frame before submitting your application.

A CTSM Program Enrollment form must be on file at the CTSM office prior to any records being archived. Once session attendance has been verified, you will be required to take the appropriate quizzes and pass with a score of 70% or better in order to receive credit for the course.

An Archive Research Fee of $125 for each conference researched is due at the time of request.
